ClearPath Health & Wellness Center was created to provide compassionate, personalized, and accessible care for individuals seeking support with their health, wellness, and life transitions. Founded by Julie Baker., DNP, FNP-C, ClearPath HWC offers a thoughtful blend of health care services and wellness coaching designed to help clients and patients feel informed, supported, and empowered.
With over 13 years of experience as a Family Nurse Practitioner, Julie brings extensive clinical expertise in urgent care, primary care, women’s health, health promotion, and basic mental health care. Her approach is warm, practical, evidence-informed, and centered on helping people feel more confident navigating their health and wellness needs.
ClearPath Health & Wellness Center offers a small-scale, patient-centered medical services in Wake Forest, NC. These services are intended for individuals seeking accessible care in a private, supportive environment.
Potential medical services may include:
- Minor illness/sick visits (18 and older)
- Perimenopause and menopause care
- Anxiety and depression care
- ADHD evaluation and management when clinically appropriate
- Weight management support
Medical services will be offered separately from wellness coaching services. Patients receiving medical care will complete appropriate medical intake forms, consent for treatment documents, privacy forms, and any required clinical documentation before receiving care.
ClearPath Wellness Center is not currently intended to replace comprehensive primary care, women's health, and emergency care. Patients experiencing chest pain, difficulty breathing, severe abdominal pain, signs of stroke, suicidal thoughts, severe allergic reaction, or any other medical emergency should call 911 or go to the nearest emergency department. Annual wellness and screening visits should be maintained by your primary care and women's health clinicians.
